Breast Augmentation

Breast augmentation is a widely performed cosmetic and reconstructive surgical procedure that increases breast size, shape, and fullness. It can help individuals gain a more proportionate and balanced figure in addition to restoring some breast volume that may have been lost because of aging, pregnancy, weight fluctuations, or following a mastectomy. Typically, breast implants made of saline or silicone are placed either under the breast tissue or beneath the chest muscles. In some cases, fat transfer to the breast may be performed as a natural option, but this approach is less frequently used.

Procedure Overview

Breast augmentation is performed with the patient under general anesthesia to minimize any discomfort and pain for the duration of the surgical procedure.

Incision Techniques

When incisions for breast augmentation are made, they are positioned in locations that help reduce visible scars, including:

  1. Inframammary: placed along the crease underneath the breast
  2. Periareolar: placed around the edge of the areola
  3. Transaxillary: made within the natural folds of the armpit

The incision type and approach are recommended by the surgeon depending on the type of implant, anatomy, and the patient’s preferences for the type of incision made.

Implant Placement Options

During surgery, the surgeon creates an exact “pocket” for the implant after incision.

  1. Subglandular Placement: Placed above the pectoral muscle and below the breast tissue. This placement allows a shorter recovery time but may look less natural for some patients. 
  2. Submuscular Placement: Placed under the pectoralis muscle. This technique can offer a more natural look and is less likely to get visible rippling; however, recovery can be somewhat longer. 

Types of Breast Implants

  1. Saline implants: Are filled with sterile salt water after insertion. The incisions can be smaller than silicone implants. Saline implants are firmer, but they can show rippling for thin-breasted patients. 
  2. Silicone implants: Are pre-filled with cohesive silicone gel. These implants are more like natural breast tissue. The FDA encourages plastic surgeons to screen for silent ruptures with MRI or ultrasound every 3 years.

Recovery and Aftercare 

Recovery from breast augmentation is a slow process that is dependent on the individual. 

-Initial Recovery: You will feel sore and swollen and have possible mild bruising for a few weeks, with the most discomfort usually felt in the first three to five days. Pain medication will normally be prescribed. 

-Activity: You should be up and walking a little while after surgery to promote circulation. Please avoid strenuous activities like heavy lifting that involve putting your arms above shoulder level for four to six weeks. 

-Support Garments: A surgical bra or supportive sports bra needs to be worn at all times to prevent the implants from moving while healing. 

-Returning to Work: Most patients with desk jobs can go back to work in 1–2 weeks. 

Final Results: The breasts will continue to soften and fall into place over the following months, with a reasonable final result seen around six months. 

Risks and considerations

Unlike many surgeries, breast augmentation surgery is usually very well tolerated, and the risk severity is relatively low with any surgical procedure. Risk factors may include: 

-Capsular contracture (thickening of scar tissue around the implant) 

-Infection, bleeding, or fluid buildup 

-Change in sensory perception of the nipple or breast 

-Rupture or shifting of the implant 

-Breast implant-associated anaplastic large cell lymphoma (BIA-ALCL) seen with textured implants 

-Systemic possible symptoms called “breast implant illness.”

Breast implants are not lifetime devices and may require replacement or revision surgery in the future.
